Informed Consent & Liability

1. I understand that I may be asked about my personal medical information and agree to participate in a discussion about my personal wellness.

2. I hereby give permission to Phaedra Rosario/bio-equanimity practitioner to perform such services – energy analysis, private sessions or classes – as I have selected.

3. I acknowledge that the Phaedra Rosario/bio-equanimity practitioner is not a counselor, psychologist, psychiatrist, or physician of any kind. I understand that Phaedra Rosario/Bio-equanimity practitioner is not licensed to diagnose illness, disease or any other physical or mental disorder. I clearly understand that this is not a substitute for a medical examination and or treatment.

4. I acknowledge that I may be asked to supply a photograph of myself or my condition. The photograph is only used to assist with the analysis and is not for publication or distribution.

5. I understand that I need to visit my personal physician for any ailments that I may be experiencing.

6. I acknowledge that no assurance or guarantee has been provided to me as to the results of the analysis, session or class.

7. I understand that if at any time I feel discomfort or have a problem with the analysis, session or class, it is my responsibility to voice my concerns.

8. I understand that payment is required at time of services offered: I agree to give 24 hour notice for cancellation to avoid cancellation fees which equal the cost of the session or class. I understand that I am responsible for paying the full fee of my session if I do not give 24 hour notice for cancelling or rescheduling a session. At any time during a session or class – I can request to stop the session or leave the class, though this may not entitle me to a refund. There is no refund for analysis.

9. If I have any health care related questions, I understand that it is recommended that I promptly call or consult your physician or healthcare provider. No information provided should be used to disregard medical and/or health related advice or provide a basis to delay consultation with a physician or a qualified healthcare provider.

10. I understand that no representation or warranty as to the reliability, accuracy, timeliness, usefulness, adequacy or suitability of the information provided to me.
